    Civic Geographies of Architectural Enthusiasm

    In this short intervention, we introduce the display and walking tour that formed our contribution to the ‘civic geographies’ exhibition and session at the Royal Geographical Society (with IBG) Annual Conference in July 2012. We then move on to explore the notion of ‘civic geographies’ in relation to the architectural enthusiasm, specifically a strong emotional attachment to buildings, exhibited by members of The Twentieth Century Society. In doing so, we suggest that a more critical account of the role of enthusiasm in the civic realm is required for two important reasons: first, because such groups can be understood to be doing geography beyond the academy in the civic realm; and second, because enthusiast knowledges and practices are active in the (re)constitution of civic landscapes of various kinds. Enthusiasts participate in caring for buildings, preserving heritage, making community spaces, as well as creating and curating local histories. An understanding of who is participating in making these civic geographies, why, how, and with what consequences, is therefore crucial

How do you copy?' Music to the amateur radio enthusiast's ears. Radio hams are the subject of Kristen Haring's enjoyable and important book, which traces ham radio culture in the USA between 1930 and 1970. Drawing on archive material, Haring composes an account as interesting to the historian of technology as to the cul- tural geographer with interests in concepts of home, leisure, masculinity and technology. Emerging `from the two interrelated processes of technical identification, creating mean- ings for technology and perceiving self in relationship to technology' (p. 161), Haring explores the technical culture of the hobbyist and argues for a closer examination of the social and personal aspects of technical communities. Enthusiasts are largely ignored by scholarship on technical culture, this volume, however, is a welcome addition to recent work on computer enthusiasts and hackers. Moreover she offers `a reminder that there exist alternative ways of using and relating to technology' (p. 18). Radio technology and its associated practices are inherently spatial, operating on a variety of scales and across a range of spaces; something explored to great effect by Haring, with the book's chapters travelling between the amateur club, the equipment manufacturer, the Federal Communications Commission and the domestic radio shack. The chapter entitled `Ham Radio at Home' is particularly noteworthy. Haring discusses here how `men's ham radio activity gained domestic acceptance' (p. 133) after being regarded for many years as a threat to social relationships and `incompatible with romantic interests' (p. 122). She examines the location of the `shack' within the home; arguing that the shacks `grounded the ethereal hobby experience and situated hobbyists in the domestic context' (p. 145). Haring provides useful insights on questions of gendered architecture, hobby space, masculine sociability and chang- ing technological values. 285 Ham radio's technical culture has a wide-ranging audience; finding a place on the bookshelves of telecommunications enthusiasts and academics alike. Haring succinctly captures the hid- den world of the radio ham, adding a charming dimension to cultural geography's current fascination with more advanced scientific and technical cultures. `di-dah-di-dah-dit' (for the uninitiated that was Morse code for `end of transmission')

    Managing enthusiasm: between ‘extremist’ volunteers and ‘rational’ professional practices in architectural conservation

    Recent geographical research has considered enthusiasm to be a shared passion and a motivator to action. Through the example of architectural conservation in Britain, and the activities of the Twentieth Century Society in particular, this paper examines the tensions between enthusiasm as a productive and positive affiliation, and enthusiasm as a negative, prohibitive, and at times extremist position. The paper makes three key contributions: firstly, it demonstrates how methodologically it is possible to trace enthusiasm, using ethnographic method to reveal not only what groups say they do, but also what they actually do. Secondly, it argues that enthusiasm is a productive but ambivalent term that creates tensions within organisations and societies where professional and volunteer roles are present. Thirdly, we show that even though enthusiasm has productive capacities, it also requires careful management, and on occasion denial. The tensions between enthusiasm and professionalism that we trace are relevant beyond the realm of architectural conservation and resonate with other groups comprised of volunteers and professionals

    Going home for tea and medals: how members of the flood risk management authorities in England construct flooding and flood risk management

    The construction of flooding and flood risk management are complex and there is potential for dissonance between the individual and institutional understanding and experience of both. In this paper we start by investigating how flooding is managed and the change in paradigm from flood defence to more adaptive approaches which embed resilience into flood risk management. Using analysis of semi-structured interviews with members of the flood authorities in England, we explore how flood management authorities construct ‘flooding’ and establish that it is often defined by in-the-moment impacts. Whilst these in-the-moment impacts are understood to be devastating, there is less appreciation of the long-term human impacts of living at risk of flooding. We uncover how the construction of ‘flood risk management’ by the flood management authorities is complicated by factors, such as the construction of resilience, the availability of funding, technical expertise, whether they provide other services to customers and the responsibility fragmentation that the Floods and Water Management Act (2010) has created. We conclude that the differing constructions of flooding and flood risk management between flood management authorities in England hinder how flooding is managed. In a world facing climate change where impacts of flooding are expected to intensify, this dissonance could readily stymie partnership working and therefore we propose that a more nuanced understanding of flooding and flood risk management is essential for effective partnership working between flood risk management authorities and communities

    Initiating and continuing participation in citizen science for natural history

    Background: Natural history has a long tradition in the UK, dating back to before Charles Darwin. Developing from a principally amateur pursuit, natural history continues to attract both amateur and professional involvement. Within the context of citizen science and public engagement, we examine the motivations behind citizen participation in the national survey activities of the Open Air Laboratories (OPAL) programme, looking at: people’s experiences of the surveys as ‘project-baed leisure’; their motivations for taking part and barriers to continued participation; where they feature on our continuum of engagement; and whether participation in an OPAL survey facilitated their movement between categories along this continuum. The paper focuses on a less-expected but very significant outcome regarding the participation of already-engaged amateur naturalists in citizen science. Methods: The paper draws upon research conducted by the authors (a sociologist of science and a cultural geographer) over a five-year period, who followed the development and implementation of the OPAL surveys. The authors engaged with members of the public and natural history enthusiasts to understand how and why people engaged with the OPAL surveys, seeking to explore the motivations and barriers they faced to any further engagement with natural history. This involved carrying out interviews and focus groups with willing participants. Results: Our main findings relate to: first, how committed amateur naturalists (already-engaged) have also enjoyed contributing to OPAL and the need to respect and work with their interest to encourage broader and deeper involvement; and second, how new (previously-unengaged) and relatively new participants (casually-engaged) have gained confidence, renewed their interests, refocussed their activities and/or gained validation from participation in OPAL. Overall, we argue that engagement with and enthusiasm for the scientific process is a motivation shared by citizens who, prior to participating in the OPAL surveys, were previously-unengaged, casually-engaged or already-engaged in natural history activities. Conclusions: Citizen science has largely been written about by professional scientists for professional scientists interested in developing a project of their own. This study offers a qualitative example of how citizen science can be meaningful to participants beyond what might appear to be a public engagement data collection exercise

    Memory Th1 Cells Are Protective in Invasive Staphylococcus aureus Infection

    Mechanisms of protective immunity to Staphylococcus aureus infection in humans remain elusive. While the importance of cellular immunity has been shown in mice, T cell responses in humans have not been characterised. Using a murine model of recurrent S. aureus peritonitis, we demonstrated that prior exposure to S. aureus enhanced IFNγ responses upon subsequent infection, while adoptive transfer of S. aureus antigen-specific Th1 cells was protective in naïve mice. Translating these findings, we found that S. aureus antigen-specific Th1 cells were also significantly expanded during human S. aureus bloodstream infection (BSI). These Th1 cells were CD45RO+, indicative of a memory phenotype. Thus, exposure to S. aureus induces memory Th1 cells in mice and humans, identifying Th1 cells as potential S. aureus vaccine targets. Consequently, we developed a model vaccine comprising staphylococcal clumping factor A, which we demonstrate to be an effective human T cell antigen, combined with the Th1-driving adjuvant CpG. This novel Th1-inducing vaccine conferred significant protection during S. aureus infection in mice. This study notably advances our understanding of S. aureus cellular immunity, and demonstrates for the first time that a correlate of S. aureus protective immunity identified in mice may be relevant in humans

    Ten-year mortality, disease progression, and treatment-related side effects in men with localised prostate cancer from the ProtecT randomised controlled trial according to treatment received

    Background The ProtecT trial reported intention-to-treat analysis of men with localised prostate cancer randomly allocated to active monitoring (AM), radical prostatectomy, and external beam radiotherapy. Objective To report outcomes according to treatment received in men in randomised and treatment choice cohorts. Design, setting, and participants This study focuses on secondary care. Men with clinically localised prostate cancer at one of nine UK centres were invited to participate in the treatment trial comparing AM, radical prostatectomy, and radiotherapy. Intervention Two cohorts included 1643 men who agreed to be randomised and 997 who declined randomisation and chose treatment. Outcome measurements and statistical analysis Analysis was carried out to assess mortality, metastasis and progression and health-related quality of life impacts on urinary, bowel, and sexual function using patient-reported outcome measures. Analysis was based on comparisons between groups defined by treatment received for both randomised and treatment choice cohorts in turn, with pooled estimates of intervention effect obtained using meta-analysis. Differences were estimated with adjustment for known prognostic factors using propensity scores. Results and limitations According to treatment received, more men receiving AM died of PCa (AM 1.85%, surgery 0.67%, radiotherapy 0.73%), whilst this difference remained consistent with chance in the randomised cohort (p = 0.08); stronger evidence was found in the exploratory analyses (randomised plus choice cohort) when AM was compared with the combined radical treatment group (p = 0.003). There was also strong evidence that metastasis (AM 5.6%, surgery 2.4%, radiotherapy 2.7%) and disease progression (AM 20.35%, surgery 5.87%, radiotherapy 6.62%) were more common in the AM group. Compared with AM, there were higher risks of sexual dysfunction (95% at 6 mo) and urinary incontinence (55% at 6 mo) after surgery, and of sexual dysfunction (88% at 6 mo) and bowel dysfunction (5% at 6 mo) after radiotherapy. The key limitations are the potential for bias when comparing groups defined by treatment received and changes in the protocol for AM during the lengthy follow-up required in trials of screen-detected PCa. Conclusions Analyses according to treatment received showed increased rates of disease-related events and lower rates of patient-reported harms in men managed by AM compared with men managed by radical treatment, and stronger evidence of greater PCa mortality in the AM group. Patient summary More than 95 out of every 100 men with low or intermediate risk localised prostate cancer do not die of prostate cancer within 10 yr, irrespective of whether treatment is by means of monitoring, surgery, or radiotherapy. Side effects on sexual and bladder function are better after active monitoring, but the risks of spreading of prostate cancer are more common

    Functional and quality of life outcomes of localised prostate cancer treatments (prostate testing for cancer and treatment [ProtecT] study)

    Objective To investigate the functional and quality of life (QoL) outcomes of treatments for localised prostate cancer and inform treatment decision-making. Patients and Methods Men aged 50–69 years diagnosed with localised prostate cancer by prostate-specific antigen testing and biopsies at nine UK centres in the Prostate Testing for Cancer and Treatment (ProtecT) trial were randomised to, or chose one of, three treatments. Of 2565 participants, 1135 men received active monitoring (AM), 750 a radical prostatectomy (RP), 603 external-beam radiotherapy (EBRT) with concurrent androgen-deprivation therapy (ADT) and 77 low-dose-rate brachytherapy (BT, not a randomised treatment). Patient-reported outcome measures (PROMs) completed annually for 6 years were analysed by initial treatment and censored for subsequent treatments. Mixed effects models were adjusted for baseline characteristics using propensity scores. Results Treatment-received analyses revealed different impacts of treatments over 6 years. Men remaining on AM experienced gradual declines in sexual and urinary function with age (e.g., increases in erectile dysfunction from 35% of men at baseline to 53% at 6 years and nocturia similarly from 20% to 38%). Radical treatment impacts were immediate and continued over 6 years. After RP, 95% of men reported erectile dysfunction persisting for 85% at 6 years, and after EBRT this was reported by 69% and 74%, respectively (P < 0.001 compared with AM). After RP, 36% of men reported urinary leakage requiring at least 1 pad/day, persisting for 20% at 6 years, compared with no change in men receiving EBRT or AM (P < 0.001). Worse bowel function and bother (e.g., bloody stools 6% at 6 years and faecal incontinence 10%) was experienced by men after EBRT than after RP or AM (P < 0.001) with lesser effects after BT. No treatment affected mental or physical QoL. Conclusion Treatment decision-making for localised prostate cancer can be informed by these 6-year functional and QoL outcomes

    GWAS meta-analysis of intrahepatic cholestasis of pregnancy implicates multiple hepatic genes and regulatory elements

    Intrahepatic cholestasis of pregnancy (ICP) is a pregnancy-specific liver disorder affecting 0.5–2% of pregnancies. The majority of cases present in the third trimester with pruritus, elevated serum bile acids and abnormal serum liver tests. ICP is associated with an increased risk of adverse outcomes, including spontaneous preterm birth and stillbirth. Whilst rare mutations affecting hepatobiliary transporters contribute to the aetiology of ICP, the role of common genetic variation in ICP has not been systematically characterised to date. Here, we perform genome-wide association studies (GWAS) and meta-analyses for ICP across three studies including 1138 cases and 153,642 controls. Eleven loci achieve genome-wide significance and have been further investigated and fine-mapped using functional genomics approaches. Our results pinpoint common sequence variation in liver-enriched genes and liver-specific cis-regulatory elements as contributing mechanisms to ICP susceptibility
